The situation
Every organisation past a certain size starts running on reconstruction.
Information sits in one system, responsibility in another, the decision in someone’s inbox, and the consequence in a report written six weeks later. Each part is recorded. The connection between them is not — so the organisation rebuilds it when it needs to act.
That cost grows when conditions change faster than fragmented software can absorb. Objectives shift. Rules shift. Capital and attention forgive less. AI increases the volume of candidate answers unless those answers meet authority, permission, and memory.

Responsibility that outlives the assignee field
Role, discipline, skills, availability, and existing commitments are institutional facts. Accountability is a relationship between a person, a role, and a matter — answerable after the fact.

Sprints, tickets, traffic, and structured breakdowns are ways of running the same underlying object. Teams keep the discipline that suits them without the organisation acquiring another system.

Permission that depends on the state of the matter
What someone may do is a function of the object, its state, and their role in relation to it. A matter in review is not editable by the team waiting on it, because that permission does not exist while it is there.
